General Information
We have a long history of teaching students across all healthcare disciplines: medical students, medical assistant interns, physician assistant and nurse practitioner students. Of course, our patient desires come first, and we respect your decision to not allow students be involved in your care should you decide that.
Our office is serviced by our physicians, nurse practitioners, medical assistants and coming soon clinical pharmacist, and case manager
Because of the volume of patients we have, we ask you arrive ten minutes before your scheduled appointment. However, here at the DMA it’s our policy to see sick patients the same day, rather than sending them away to an Urgent Care or ER, medical emergencies take priority and there are times patients must be taken out of order.
We ask that you give us a 24 hour notice for cancellations, as this allows us to give that appointment to those who are waiting.
Payment and Insurance Information
Payment and Insurance Information – Payment is expected at the time of visit. We accept most major forms of insurance and credit cards. If you cannot pay, please let us know, as we can arrange for a payment plan.
Please have your most current insurance card with you. We are required to verify your insurance and to collect a co-payment, if required.
Items to Bring to Your Appointment
- Valid picture ID
- A current copy of your medical insurance card
- A list of all medications and any over-the-counter drugs or herbal supplements that you are currently taking, including strength and dosage
- For children, a current record of immunizations
- If you have one, a current copy of your Durable Power of Attorney for Health Care and/or Living Will document
